Is the parenting agreement legally binding?
Not on its own. The template gives you a complete, clearly written agreement that both parents can sign. To make it enforceable it usually needs to be submitted to the court handling your case or incorporated into a consent order. The completion guide explains how that works and what to ask an attorney licensed in your state.
Does it work if my co-parent will not cooperate?

Is this specific to one state?
